Filling your prescription via www.hoopershealth.com is easy, convenient and private. Please note that only Prescriptions written by a Physician licensed in Ontario, Canada can be accepted. If you have a new prescription that you would like to have filled. Please use one of the following methods:

1. Mail the original prescription to:

2. Have your doctor Fax the prescription to:

Transferring a prescription from another pharmacy to ours is quick and hassle free. Simply provide us with the following information via the contact information above.

• Patient's First & Last Name

• Patient's Telephone Number

• Pharmacy Store Name

• Pharmacy Telephone Number

• Medication(s) to be transferred, including Rx number and name of medication

• Hooper's location you want your Rx to be transferred to ( Oakville not applicable ).
