Medication Reviews  
Do you know what all your medications are used for and how to use them?
Are you being looked after by more than one doctor? Are you concerned that the different doctors looking after you may not all have the same information? Do you think that you are taking too many medications?

Call or contact the Hooper's Pharmacy most convenient to you and book an appointment for a medication review. We can either have you bring all your medications into our pharmacy, or if you prefer our pharmacist will come to your home. (Some restrictions apply). 

After the medication review, our pharmacist can discuss your concerns with the other health professionals involved in your care. For example, if there are medications that our pharmacist feels should be discontinued or changed he or she will discuss the possible changes with your doctor. Also, with your permission, we will discuss your medication review with your other healthcare providers, such as, Nurse or Diabetic educator.
